Construction Company Cited After Crane Operator Dies in Rollover Accident

Crane operator was not wearing safety restraint when rough terrain crane slipped into a ditch, overturned and crushed operator as it rolled off an embankment

The U.S. Department of Labor's Occupational Safety and Health Administration (OSHA) issued Wanzek Construction, Oakdale, Neb., with one serious safety violation for exposing workers to struck-by hazards following an investigation into the death of a 40-year-old crane operator on May 27, 2015.

The worker was not wearing a safety restraint while operating a 90-ton rough terrain crane which slipped into a ditch, overturned and crushed the operator as it rolled off an embankment. He had been employed on the jobsite for only 10 days.

The serious safety violation carries a proposed $7,000 penalty.
