R&B Contractors is facing $116,900 in OSHA fines and 15 safety violations after a worker fell 40 feet from a roof and died. OSHA investigators found there were no guardrails, safety nets or personal fall prevention devices where the workers were working on a low slope commercial roof.
OSHA issued the contractor one willful, 11 serious and three other-than-serious safety citations.
OSHA said the contractor also failed to train workers about fall hazards, develop a safety and health program, designate a safety monitor, train powered industrial vehicle operators or record injuries and illnesses.
