Falls continue to play a prominent role in the Occupational Safety & Health Administration's (OSHA) list of violations during the first three months of 2019. However, the top fine for the first quarter stemmed from an electrical injury violation.

Here are the top 8 OSHA fines for the first three months of 2019:

  1. T.D. Fraley & Sons — Alexandria, VA; total proposed fines of $528, 692 after a working breaking down scaffolding cam into contact with an overhead power line
  2. Crown Roofing LLC — Sarasota, FL; total proposed fines of $265,196 for fall protection hazards
  3. Frame Q — Palisades Park, NJ; total proposed fines of $261,451 for fall protection violations
  4. Five Star Commercial Roofing — Hartford City, IN; total proposed fines of $182,000 for fall protection violations
  5. PCL Construction Services — Orlando, FL; total proposed fines of $144,532 for not inspecting formwork, shoring, working decks and scaffolds after two workers were killed when supports collapsed
  6. Sonny Steel Erectors — Houston; total proposed fines of $116,689 for fall protection violations
  7. CV Construction LLC — Akron, Ohio; total proposed fines of $103,468 for fall protection violations
  8. Muska Electric — Roseville, MN; total proposed fines of $100,000 after an employee died from electrocution
