Construction Company Facing $42,000 in Penalties After Trench Collapse Killed Worker

Investigation reveals Kentucky contractor had not trained workers in rescue procedures of any kind

In June 2015, two employees of Josh Lefevre Construction Company in Kentucky got caught in a trench collapse in a 10-foot-deep sewer line trench. One employee died from the accident. The proceeding investigation found the contractor had not trained its workers in rescue procedures of any kind.

The workers in the trench were also not wearing hard hats and did not have ladders in place to get out of the trench. In addition, the trench box did not provide enough protection as it was four feet shorter than the hole.

The contractor is facing eight separate penalties and total of $41,600 in fines from the Kentucky Labor Cabinet. 

The penalties include no isolating a manhole, not testing the space before workers entered, no rescue equipment on hand and lack of training.
